The Lower Eastern Shore Tri-Counties of Maryland: Somerset, Wicomico, and Worcester have united
together to help lower and prevent the number of Tri-County residents with diabetes. Tri-County residents are being diagnosed with diabetes at an alarming rate, greater than the normal rate of the rest of the United States. Health professionals continue to recognize the need in the area for targeted diabetes awareness, prevention, and educational programs.
The unification of the Tri-County Diabetes Alliance has been in existence since 2004. The Tri-County Diabetes Alliance is a joint effort between the local health departments, health care professionals, and community volunteers.
The Mission of the Tri-County Diabetes Alliance is to raise awareness and improve the health of people with prediabetes and diabetes by creating and sustaing an effort to inform, educate and assist individuals to improve their health.
The following statement of objectives for the Tri-County Diabetes Alliances will follow the Healthy People 2020 guidelines:
D HP2020-14: Increase the proportion of persons with diagnosed diabetes who receive formal diabetes education.
D HP2020-15: Increase the proportion of adults with diabetes whose condition has been diagnosed.
D HP2020-16: Increase prevention behaviors in persons at high risk for diabetes and those with prediabetes.
