Getting Started - Managing Your Condition

Once you've been diagnosed with diabetes or pre-diabetes, it is important to learn as much as possible about managing your condition. Your diabetes health care team should include at  least your doctor, a certified diabetes educator, and a registered dietitian. Other members to consider having on your team include a podiatrist, optometrist, dentist, and endocrinologist.   Together they can teach you about managing your diabetes on a daily basis. They will teach you about:

  • Eating healthy
  • Staying physically active
  • Monitoring your blood sugar
  • Monitoring your eyes, your vision, and your feet
  • Taking your medicine (If prescribed)
  • Setting healthy goals
  • Problem solving

Get Your Regular Exams and Tests Recommended By The American Diabetes Association For People With Diabetes Listed Below.

Exams or Tests When
Blood Pressure Every office visit
Weight Every office visit
Foot Exam Every office visit
Blood Glucose Record Discuss every office visit
Dentist Check-Up Two (2) times a year
A1C Blood Test - ( Average blood sugar value during past two to three months) Two (2) to four (4) times a year based on blood sugar control an/or medicine changes.
Blood Tests for Cholesterol, HDL, LDL, and Triglycerides Every year (more often if levels not in desirable range)
Dilated Eye Exam by ophthalmologist or optometrist Every year
Foot Exam by a podiatrist Every year
Urine Check for Protein/Micro albumin and Creatine Ratio Every year
Flu Shot Every year
Pneumonia Vaccine As per advice by your health care provider
